Birth Certificate

Home Birth: birth certificates and social security numbers

It is important to send in birth certificate paperwork within 30 days after the baby is born. Talk to your midwife as she may have resources to help you do this.

If the 30 day window is missed, parents will need to make an appointment with the Department of Health to apply for a birth certificate. Both mother and child will need to be present to apply.

Hospital Birth: birth certificates and social security numbers

Before you leave the hospital you can complete a birth certificate and request a Social Security number for your baby at the same time. For questions regarding a newborn's paperwork, please call (808) 242-2352
